Card On File Authorization Form
If you would like to have your card on file for making copays or account payments, please complete our Card On File Authorization Form. We will never charge your card without your permission. This option simply allows you to pay your copay at your visit should you not have your wallet. It also allows you to make a payment on your account over the phone without having to provide your credit card information aloud.
Photo/Video Consent & Release Form
Interested in helping us? Complete our Photo/Video Consent And Release form to allow us to take pictures or videos of treatment methods utilized during your physical therapy experience. We will always verbally ask for your permission prior to capturing any footage!
Telehealth Consent Form
If any of your physical therapy sessions are performed remotely, you must sign our Telehealth Consent form. We cannot treat you in this manner without it.
